HOW CAN I DONATE MY EYES ?
Print, fill out and sign the Donor Card you see below or sign the back of your driver's license. Have two witnesses sign it, and then carry the card with you at all times. The gift of your eyes need not be included in your will; eyes must be procured and used before wills can be probated.
Tell your relatives, physician and or clergy of your intentions. This is very important because to make sure family wishes are honored, eyes will not be removed without permission from the next of kin, even when the deceased person carries a signed donor card.
